About Us

The Northwest Colorado Community Health Center is a Federally Qualifed Health Center serving Moffat County. We deliver quality and affordable primary health care to Moffat County residents. The clinic has been in operation for many years has been operated by the Northwest Colorado Visiting Nurse Association since 2005.

Our success is our partnerships with our patients and community. We are proud of our achievements and we look forward to continuing to build our capacity in order to serve our growing community.

The Health Center is open daily and schedules appointments Monday through Friday. To schedule your appointment call 970-824-8233.

Hours of Operation

Our clinic is open Monday from 8am to 7pm and Tuesday though Friday 8 am to 5pm. After hours and on weekends Health Center clients have access to professional services by calling the Health Center number.
